Captain Mitchell died on October 26, 2009, at Forward Operating Base Dwyer in Helmand Province, Afghanistan. According to the Defense Casualty Analysis System, he was killed in action. The Department of Defense stated he died while supporting combat operations, though the precise tactical details of the incident were not publicly detailed in major news reports.
